Using the online versions of Academic Performance Notice and Referral/Documentation forms

These forms are now set up as interactive Acrobat PDF files that will open in either your web browser or in Acrobat Reader, depending on how your computer is configured.  You may fill out the form, and click the Send Form button to submit the information.  Your email program should open a message addressed to Student Affairs, with the information you entered in the fields sent as an attachment.  Student Affairs will send a copy of the Notice to the student involved, in the case of the Academic Performance Notice, and keep a copy of the notice or referral on file.

I. ACADEMIC PERFORMANCE NOTICE

Use the Notice option when you are concerned about a student’s academic performance. The Notice is a direct way to alert a student of your concern and provide written suggestions for improvement. Since a copy of the Notice also goes to Student Affairs, it provides us a way to identify students who are at risk.

The Academic Performance Notice is now online at http://www4.cord.edu/stuAffairs/referrals/PerformanceNotice.pdf.

To allow additional opportunities for interventions/assistance, copies of the Notice sent to the student are also sent to the student's advisor, the Academic Enhancement Center, International Student Advisor (if applicable), and the student's coach if he or she is involved in Athletics.

NOTE: Advisors may place copies of Notices in advisement files.

II. REFERRAL/DOCUMENTATION TO THE STUDENT AFFAIRS OFFICE

Use the Referral/Documentation option when you have more serious concerns about a student. Examples of what to refer or document include the following: (1) Concerns about a student’s academic performance and/or well being that warrant a contact of the student by a member of the Student Affairs staff. (2) Notification that a student has stopped attending class but has not taken steps to drop the class (student’s name still appears on class list). (3) Concerns and/or information that you believe should be brought to the attention of the Student Affairs Office. Referral information may be of a personal or confidential nature. (4) Documentation of actions or steps you have taken to assist a student (e.g., referred student to Counseling Center).

Your options to make a referral are to:

  1. Submit the online form at http://www4.cord.edu/stuAffairs/referrals/ReferralForm.pdf; or
  2. Complete the “Referral/Documentation to the Student Affairs Office” form available from the Student Affairs Office and send it directly to the Student Affairs Office—Attention Lois Cogdill; or
  3. Call the Student Affairs Office at 3455.

If you refer a student, a Student Affairs staff member will provide follow-up information to you regarding the outcome of your referral.
